PACES: Patient Cases and Profiles

Patients

We pride ourselves on the quality and range of patient cases we make available to our candidates. Chosen according to strict criteria to ensure complete exam relevance, our patients are spread across the four clinical stations. Each of our patients attends only one day of the course, so there will be no repetition, and you'll see a total of 120 patients over a four day course.

Patient Profiles

Name: Samuel Masterson

Age: 83

History: Man has shortness of breath on exertion.

Question: Please examine his cardiovascular system and establish the cause of his dyspnoea.

Physical Signs: He has limited scleroderma (crest syndrome), with moderate mitral regurgitation, mild mixed aortic valve disease (gradient 37mmHg), and mild pulmonary hypertension (pulmonary systolic pressure 46mmHG)

Diagnosis: Crest mixed aortic valve disease and mitral regurgitation

Name: Ivy Williams

Age: 74

History:  Lady has difficulty walking.

Question: Please examine her legs neurologically in order to establish a diagnosis.

Physical Signs: Nerve biopsy scar , R lateral malleolus, weak ankles, stocking anaesthesia, absent ankle jerks, downgoing plantars, cerebellar signs.

Diagnosis: Peripheral neuropathy secondary to vasculitis. Lower motor neurone signs in legs.

Name: Robert Adamson

Age: 59

History:  Had noted distension of his abdomen.

Question: Please examine his abdomen and discuss the possible underlying diagnoses. Following a recent accident, this man has tingling in his fingers. Please examine his upper limbs.

Physical Signs: has stopped drinking, but new signs after fall = cervical spine injury.

Diagnosis: Alcoholic Liver Disease, hepatomegaly, oedema, pacemaker.
